Just stop by the Health Service Director’s office and they’ll be happy to answer your questions and help you through the process. We’re here to support your recovery every step of the way!
Step 1: Your Health Services Director (HSD) will assist you in getting a doctor’s order from your physician, which will be valid for 30 days
Step 2: Gather and submit required documentation and verify insurance coverage (Note: this verification can take up to two weeks)
Step 3: Schedule your personal assessment, where you therapist will talk with you about your therapy goals and create a personalized treatment plan
